An Ambulance Victoria helicopter was required to lift the woman from the scene.
"They had to get the chopper to winch some paramedics down to help her," Stawell SES duty officer Jenny Rooke said.
"They had to get her up to a clear spot near the Pinnacle to do so. Her shoulder injuries must have been more severe than originally thought."
UPDATE - 2.45pm
Emergency services have been called to the Grampians for the second time in a day.
Stawell SES volunteers are on their way to assist a 48-year-old female who sustained a shoulder injury after a fall near the Pinnacle at about 1.45pm.
"About 200 metres from the top of the Pinnacle she has fallen and hurt herself. We believe she is alone but is otherwise safe," Stawell SES duty officer Jenny Rooke said.
"Our members will meet with police and ambos to help her."

Emergency services were called to assist an "elderly woman" believed to have been hurt following a fall in the Grampians on Tuesday morning.
Stawell SES and Ambulance Victoria members responded to calls of a woman aged in her 70s falling at Clematis Falls after about 11am.

The woman potentially sustained injuries in the incident, meeting emergency services near the Chatauqua Peak trail.
Stawell SES duty officer jenny Rooke said the woman did not sustain major injuries and was being assessed by Ambulance Victoria members.
"We had everything ready to carry her put but she was able to just walk back to her vehicle," she said.
"It was best to make the call anyway just in case."
