The teams will meet at 11.30am on Sunday, July 2 at Melville Oval, Hamilton to decide the fifth and sixth places.
Horsham defeated Stawell by 14-points earlier in the season.
Hamilton Kangaroos and Horsham Saints will feature in the inaugural grand final at 2.30pm.
AFL Western District Commission Chair Greg Wood lauded the league’s first season as a “huge success”.
“It’s been a really popular league and we’re hoping to build on it next year,” he said.
The league best and fairest count will be held at the Hamilton Exhibition and Conference Centre at 4.15pm where the best and fairest, runners up and leading goal kicker awards will be presented.
