Police say the mini-bus driver was one metre over the centre line when he struck the motorcycle about 5pm three kilometres east of Port Campbell on the Great Ocean Road.
The motorbike rider, a 30-year-old man from the Werribee area, and his 50-year-old female pillion passenger, from South Morang, are being flown to Melbourne hospitals in separate helicopters.
One is going to The Alfred Hospital and the other to the Royal Melbourne Hospital.
They are both in critical conditions with life threatening injuries.
The man has severe injuries to his right arm and right leg while the woman has severe injuries to her right leg.
They were in a group of five motorcycles on a social outing when the accident happened.
Police say it was extremely fortunate only one of the motorcycles was involved in the collision.

The mini-bus driver was the sole occupant of the bus.
