The Melbourne-duo left for a hike along the Pinnacle Trail just before sunset on Saturday.
"With light fading quick they became lost, disoriented and got off track," Sergeant Robert Bartorelli said.
"Between SES, police and the VicPol air wing we were able to locate them late at night and got them out at around 11.45pm.
"We were able to locate their position but the terrain made it impossible to get to them, so they had to be winched out by helicopter."
Sergeant Bartorelli said people looking to hike in the Grampians need to ensure they are prepared and think through their situation.
"They set out just before dark and were quite ill-prepared. They had no head lamps or torches, had a lack of warm clothing and overall weren't prepared for the conditions they set out in.
"When we got them back they were affected by the elements, it had gotten quite cold by that time of night.
"It's just another reminder to people to have a plan when heading into the Grampians for a hike."
