Victoria Police said a van was travelling along the Wimmera Highway when a truck began to overtake it shortly before 4pm.
The van, which tried to make a right-hand turn and collided with the truck, ran off the road and rolled, coming to rest in a paddock.
A male passenger was thrown from the van. He was airlifted to hospital in a serious condition.
Police said the van driver and another passenger were taken to hospital with minor injuries while the truck driver was not injured.

UPDATE 5.15pm: Ambulance Victoria has confirmed one person has been taken to Bendigo hospital. Meanwhile, the air ambulance has arrived at the scene.
EARLIER: Emergency crews are responding to an accident near St Arnaud and three people are being assessed at the scene.
Ambulance Victoria and Victoria Police are responding to reports of a vehicle rollover 30km outside of St Arnaud near the Wimmera Highway.
The air ambulance has been called and is on its way.
