Emergency services personnel worked for more than 30 minutes to free an elderly occupant of one of the vehicles, after she became trapped following the accident at about 1pm.
The woman, aged in her 70s, was transported by ambulance to Ballarat Base Hospital where she was treated for serious but non life threatening injuries.
The elderly woman was the front side passenger of a silver sedan travelling east on the highway when it was struck by another vehicle travelling west.
Her husband, the driver of the vehicle and also aged in his 70s, was taken to hospital for treatment.
The collision is believed to have occurred after the driver of a white sedan lost control of her car and crossed to the wrong side of the highway.
The 49-year-old woman, along with her two-year-old grand daughter passenger, were also taken to hospital for treatment.
Northern Grampians Highway Patrol Senior Constable Michael Petrov said police suspect fatigue may have played a part in the crash.
The Western Highway at Great Western was closed for almost two hours with traffic diverted along Stephensons Road.
